End-of-life wishes take on many forms. Some examples of this essential and meaningful work:
Casting: There is power, importance, and a sense of connection through holding hands. Have this sensation forever with a bronzed cast of hands.​
Fingerprints: Gift your fingerprints to loved ones. These unique and personal prints can be made into memory jewelry and tattoos.
Legacy work: Complete a full circle in life's journey, and we will guide you through the process of leaving some of your 'selves'—your experiences, ideas, values, and stories. These will not just be memories but a part of your legacy that will live on in the minds of your loved ones, bringing them comfort and reassurance.
To meet eligibility criteria, you must have a prognosis of one year or less and reside within Southern Georgian Bay
From the very start, Hospice Georgian Triangle has been a trusted partner providing compassionate care in our community. As of May 1, 2025, we have started transitioning the Living Wish program into their capable hands to create a sustainable path forward that ensures everyone in South Georgian Bay can experience the power of one final, meaningful wish.

Please note that by applying here, you are consenting to sharing this application with the Hospice Georgian Triangle wish coordination team, and Hospice Georgian Triangle wish coordinators will fulfill all wish requests submitted after May 1st, 2025.
